ABSTRACT

Se realizó un estudio descriptivo y transversal de 73 pacientes con fibrilación auricular de nueva aparición en las primeras 72 horas del periodo posoperatorio de revascularización coronaria, intervenidos quirúrgicamente en el Servicio de Cirugía Cardiovascular del Hospital Provincial Docente Clinicoquirúrgico "Saturnino Lora Torres" de Santiago de Cuba, desde enero de 2010 hasta igual período de 2013. En la serie, los mayores de 60 años fueron los más afectados (71,2%), con un aumento en la frecuencia a medida que avanzó la edad. Primaron el sexo masculino (74,3 %) y el hábito de fumar (86,0 %). Entre los factores más notables figuraron: antecedentes de hipertensión arterial, fracción de eyección ventricular izquierda por debajo de 55,0 %, revascularización incompleta y uso del bypass de apoyo. Aunque esta arritmia se relacionó con otras complicaciones posoperatorias, el bajo gasto cardiaco y los accidentes vasculares encefálicos predominaron en esta investigación.


A descriptive and cross-sectional study was carried out in 73 patients with new-onset atrial fibrillation in the first 72 hours of the postoperative period of coronary bypass surgery, surgically treated at the Cardiovascular Surgery Department of "Saturnino Lora Torres" Provincial Teaching Clinical Surgical Hospital in Santiago de Cuba, from January 2010 to the same month of 2013. In the series those older than 60 years were the most affected (71.2%), with an increase in frequency as age advanced. Males (74.3%) and smoking habit (86.0%) prevailed. Among the most remarkable factors were history of hypertension, left ventricular ejection fraction below 55.0%, incomplete revascularization and use of the bypass support. Although this arrhythmia was related to other postoperative complications, low cardiac output and strokes prevailed in this study.

ABSTRACT

Se realizó un estudio descriptivo y prospectivo de 112 pacientes colecistectomizados por videolaparoscopia con anestesia general orotraqueal en el Hospital Clinicoquirúrgico Docente "Dr. Joaquín Castillo Duany" de Santiago de Cuba, desde diciembre del 2010 hasta marzo del 2011, a fin de determinar la incidencia de complicaciones asociadas a la extubación traqueal en el periodo posoperatorio inmediato. En la serie predominaron los afectados de 31-45 años (43,7%) y la taquicardia (19,6 %) como la complicación más frecuente. Asimismo, la edad, la obesidad y el tabaquismo favorecieron el desarrollo de complicaciones atribuibles a la extubación traqueal, las cuales tuvieron mayor frecuencia en la medida que el paciente estuvo más despierto al realizar la misma.


A descriptive and prospective study was carried out in 112 patients undergoing cholecystectomy by video laparoscopy under general orotracheal anesthesia in "Dr. Joaquín Castillo Duany" Teaching Clinical Surgical Hospital of Santiago de Cuba, from December 2010 to March 2011, to determine the incidence of complications associated with tracheal extubation in the early postoperative period. In the series patients of 31-45 years (43.7%) and tachycardia (19.6 %) as the most frequent complication prevailed. Also, age, obesity and smoking habit favored the occurrence of complications attributable to tracheal extubation, which were more frequent as the patient was more awake when performing it.

ABSTRACT

PURPOSE: Sentinel lymph node biopsy (SLNB) is a relatively accurate diagnostic method for determining the presence of axillary lymph node metastasis (ALND). SLNB reduces the need for axillary lymph node dissection, thereby decreasing the postoperative axillary morbidity. The present study compared the postoperative axillary morbidity rates during early postoperative days for patients undergoing either SLNB or conventional ALND. METHODS: We conducted a prospective case-control study of breast cancer patients. The degree of axillary morbidity was compared between 28 SLNB patients (Group I) and 38 ALND patients (Group II). RESULTS: The SLNB group showed decreased arm swelling and restriction of their shoulder motion in comparison with the conventional axillary dissection group (p<0.05). SLNB and additional lymph node sampling did not result in any additional morbidity. CONCLUSION: SLNB or lymph node sampling was associated with less axillary morbidity like arm edema, limitation of motion than was conventional ALND. The rate of postoperative axillary morbidity did not differ following lymph node sampling and SLNB. SLNB may be an effective method for diagnosing of axillary lymph node metastasis with decreasing the postoperative axillary morbidity

ABSTRACT

Objectives: To investigate the value and characteristics of nutritional support in therapy for patients with early postoperative inflammatory small bowel obstruction(EPISBO). Methods: The nutritional support was used in the treatment of EPISBO of 35 patietns. TPN was provided through peripheral vein or central vein and was followed by EN or oral diet. Results: Nutritional support decreased the complication of EPISBO and shorten the course. Conclusions: Rational nutritional support therapy is effective and important in the treatment of EPISBO.

ABSTRACT

In experimental sutureless muscle surgery with tissue adhesives, current materials have inadequate adhesive strength in the early postoperative period.We performed a modified muscle surgery using a stitch combined with Tisseel (Immuno AG, Vienna, Austria)in order to evaluate the adhesive strengh as a replacement for conventional suture techniques in muscle surgery. Thirty eyes of 15 rabbits were used.We recessed superior and inferior rectus muscles by three different methods, Tisseel only, a stitch with Tisseel, and a stitch only, in each group of 5 rabbits.Conjunctival incisions were closed with the remaining Tisseel in 7 rabbits, and with vicryl in 8 rabbits. Tensile strength of the scleral reattachment site was measured and conjunctival closure was examined by slit-lamp biomicroscopy, at intervals of 1 hour, 12 hours, 1 day, 3 days and 5 days. The strengths of a stitch with Tisseel at each interval were 83.25 +/-1 4 .0 6 gm, 137.50 +/-22.88 gm, 169.75 +/-23.95 gm, 151.50 +/-41.99 gm, and 265.50 +/- 25.01 gm, respectively.The strengths of Tisseel alone were 61.50 +/-2 0 .2 1 gm, 101.50 +/-11.00 gm, 113.25 +/-28.69 gm, 120.50 +/-18.36 gm, and 222.75 +/- 57.67 gm.They were 52.25 +/-24.85 gm, 89.50 +/-16.05 gm, 130.75 +/-21.98 gm, 1 5 3 .7 5 +/-30.35 gm, and 261.50 +/-60.47 gm at each interval in rabbits with a stitch alone. These results showed that the tensile strengths of a stitch with Tisseel were stronger than those of the other two methods up to 1 day after surgery.Even at postoperative 12 hours, the strength was more than 130 gm which was strong enough for scleral attachment.All of the conjunctival closures with Tisseel were well maintained without any complication up to 5 days, the same as vicryl sutures. In conclusion, this study revealed that muscle attachment to the sclera by a stitch with Tisseel was simpler and safer than classical sutures with reduced risk of sclera perforation.Its strong tensile strength at early postoperative days suggests that the method might be considered as an alternative method to classical reattachment using suture material and also that conjunctiva could be closed with Tisseel remnant.
